• Identification Card: Driver's license or a state issued ID
• Insurance Card: Most recent insurance card. Insurance verification is recommended prior to your appointment. Co-pays are due at the time of service.
• Forms: Forms can be completed online, printed and signed for legibility and ease.

• List of current medications including name, dose and frequency of medication.
• List of other physicians you are currently seeing and their contact numbers
• Recent medical records, labs, x-rays etc.
• List of current medications
• List of any allergies

We recommend that new patients arrive for their appointment approximately 15-20 minutes before the scheduled appointment time.

New patients may require 30-60 minutes for their initial evaluation depending on your medical complexity. Having all the above items, and completed forms will save you time during your visit.
